Messages may include attendance and tardy updates, academic reminders, adult-connection check-ins, teacher announcements, Saturday School information, school events, Parent Needs Surveys, 5-Star Students recognition, incentives, rewards, and available student or family support.

• Providing consent for optional email communication is voluntary.

• Consent is not a condition of enrollment, instruction, grades, or access to school services.

• Ordinary email may not be fully secure; sensitive student information should be discussed using a school-approved secure method or direct conversation.

• The student may reply to a message, ask for assistance, or request contact with a trusted adult. • This authorization does not permit advertising, unrelated commercial messages, or communication through personal social-media accounts.

The information collected through this form will be used by Mountain View High School and authorized school personnel to:

• Manage school-family communication preferences
• Support attendance and on-time arrival
• Strengthen student engagement and adult connections
• Provide academic, attendance, and family support
• Send school-event and Saturday School information
• Recognize student progress through 5-Star Students
• Document consent, communication attempts, and family responses
• Evaluate and improve the 30-Day Student Engagement System

Information will be limited to authorized educational and administrative purposes. Access should be restricted to personnel and approved service providers who need the information to perform authorized duties.

The information will not be sold or used for unrelated advertising or commercial profiling.

Email, text messages, telephone calls, automated voice messages, and voicemail may not be fully secure. Sensitive educational, disciplinary, counseling, disability, medical, or legal information should be communicated through a school-approved secure method or direct conversation.

VOLUNTARY CONSENT

I understand that:

• My communication selections are voluntary.

• Refusing optional email, SMS, telephone, voice, or direct-student communications will not affect ’s enrollment, instruction, grades, participation, or access to school services.

• I may update my contact information or communication preferences at any time.

• I may withdraw consent for future optional communications by following the opt-out instructions in a message or contacting Dr Jordan B Smith Jr.

• Withdrawal will apply to future optional communications and will not invalidate communications previously sent.

• Emergency, safety, legally required, or other communications authorized by law or school policy may be handled separately.

• I am responsible for notifying the school if my telephone number, email address, guardianship status, or communication authority changes.
